For a D Visa no insurance is required. However for TRP insurance is required. When I got my TRP local Ovir sold me a one year medical insurance "for expats" by vienna insurance at 550 UAH.
In July 2015 I applied for Visa D in Lublin Poland, where they insisted on a health insurance. It took me quite some effort to explain them that my actual insurance is valid worldwide as such. Still, I have to admit that at that time there was a woman with an iron face 'serving'. No English, no friendliness, no "nothing" ;(
Did not know then Krakow is apparently much better service which I do now thanks to this Forum  ;D

A young, professional and very competent dentist - About $50 USD, incl. the x-rays.

Glad it went well.  Give the doc a little publicity if it went well.
